The MAES Animal Use Agreement requires the following information:

  • Date of Request
  • Project Start/End Date
  • Type of use (research project, class use, club use, etc.)
  • Project Title and Project Leader Name
  • Approval Number/Date from MSU-IACUC and MSU-AACUC
  • Other Project Personnel Names/Phone Numbers
  • Animal Requests (species, number of animals, sex of animals)
  • Will animal be euthanized? If no, will animal be able to be immediately turned back into herd/flock?
  • Services and Resources Provided by MAES Personnel (labor, feed, facilities, equipment, etc.)
  • Index Number for Billing
  • Signatures of Project Leader, Department Head and Animal Operations Manager
